It is UWA policy that assessments are returned to students within 4 weeks of due date or assessment date. If you have an assessment that has taken longer than 4 weeks to return, please fill in this form so that WAMSS can endeavour to follow it up.
If you have previously submitted this information to your year reps you don't need to do so again.
